By virtue of an affidavit sworn before Executive Magistrate BHUBANESWAR on Dated 19th February 2025,I, Mrs. RUPA NAYAK, Address AT-JARIPUT,PO-GUDUM,PS-KHORDHA,DIST- KHORDHA,PIN-752056, Odisha, have changed my Son's name from "SUBHA NAYAK" to "SAI SIDHARTH NAYAK". Henceforth, He shall be known as "SAI SIDHARTH NAYAK" for all purposes.
