By virtue of an affidavit sworn before the Executive Magistrate Salipur on Dated 20th February 2025, I, Mr. Basudev Swain, Address At-Balisahi, Po-Salipur, Ps-Salipur ,Dist-Cuttack,Odisha 754202, Odisha, have changed my name from "Basudeb Swain" to "Basudev Swain". Henceforth, I shall be known as "Basudev Swain" for all purposes.
