By virtue of an affidavit sworn before the Executive Magistrate BHUBANESWAR on Dated 6th May 2025, I, Mr. RABI MADHI, Address AT/PO-TUMUSAPALLI, DIS-MALKANAGIRI, PS- MALKANAGIRI, Odisha, have changed my name from "SRIRABI MADHI" to "RABI MADHI". Henceforth, I shall be known as "RABI MADHI" for all purposes.
