By virtue of an affidavit sworn before Executive Magistrate GOP on Dated 4th January 2025,I, Mr. PRAKASH KUMAR NAYAK, Address VILLAGE/PO-GANESWARPUR ,PS-GOP ,DIST- PURI ,PIN-752110, Odisha, have changed my Daughter's name from "PRANGYASHRI NAYAK" to "PRAJNASHREE NAYAK". Henceforth, She shall be known as "PRAJNASHREE NAYAK" for all purposes.
