By virtue of an affidavit sworn before the Executive Magistrate Berhampur Ganjam on Dated 11th February 2025, I, Mrs. Putti Rajami, Address AT-Raghunathpur, PO-Narendrapur, PS- ChamaKhandi, Dist-Ganjam, Pin-760007, Odisha, have changed my name from "P Rajamma Reddy" to "Putti Rajami". Henceforth, I shall be known as "Putti Rajami" for all purposes.
