By virtue of an affidavit sworn before the Executive Magistrate JAJPUR on Dated 20th March 2025, I, Mrs. Sarthi Boity, Address AT: DUDURANTA, PO:PARIPADA NIJAMPUR, PS: Mangalpur, DIST: JAJPUR, PIN: 755011, Odisha, have changed my name from "SARATHILATA BOITI" to "Sarthi Boity". Henceforth, I shall be known as "Sarthi Boity" for all purposes.
