By virtue of an affidavit sworn before the Notary Public Sambalpur on Dated 13th March 2025, I, Mrs. Purnima Bhoi, Address At-Kankhinda, Po/ps-Sason, Dist-Sambalpur-768200, Odisha, have changed my name from "Koushalya Bhue" to "Purnima Bhoi". Henceforth, I shall be known as "Purnima Bhoi" for all purposes.
