By virtue of an affidavit sworn before Executive Magistrate TALCHER on Dated 25th August 2025, I, Mr. Jogeswar Rout, Address At-Basulei,Po-Gengutia,Ps-Parjang,Dist- Dhenkanal,Odisha,759146, Odisha, declare that I have changed the name of my Daughter from "Poonam Rout" to "Kritika Rout". Henceforth, She shall be known as "Kritika Rout" for all purposes.
