By virtue of an affidavit sworn before Executive Magistrate Erasama on Dated 16th April 2025, I, Mrs. Kanakalata Mandal, Address At/Po-Kunjakothi, Ps-Erasama Dist-Jagatsinghpur PIN- 754139, Odisha, declare that I have changed the name of my Son from "Sahil Mandal" to "Rajkumar Mandal". Henceforth, He shall be known as "Rajkumar Mandal" for all purposes.
