By virtue of an affidavit sworn before Executive Magistrate JAGATSINGHPUR on Dated 3rd June 2025, I, Mr. DHANESWAR SENAPATI, Address AT-DARADA,PO/VIA-BORIKINA,DIST- JAGATSINGHPUR,754110, Odisha, declare that I have changed the name of my Son from "SMRUTIRANJAN SENAPATI" to "SMRUTI RANJAN SENAPATI". Henceforth, He shall be known as "SMRUTI RANJAN SENAPATI" for all purposes.
