By virtue of an affidavit sworn before the Executive Magistrate Betnoti on Dated 21st April 2025, I, Mr. LAKSHYAHIRA SUKULA, Address AT-RAIBANIA, PO-RAIBANIA, PS-RAIBANIA, DIST- BALASORE, PIN-756033, Odisha, have changed my name from "ASHOK SUKULA" to "LAKSHYAHIRA SUKULA". Henceforth, I shall be known as "LAKSHYAHIRA SUKULA" for all purposes.
