By virtue of an affidavit sworn before Executive Magistrate PARJANG on Dated 28th April 2025, I, Mr. RAJESWAR PARIDA, Address AT/PO-KANTOR, PS-PARJANG, DIST-DHENKANAL, ODISHA, PIN CODE-759120, Odisha, declare that I have changed the name of my Daughter from "KRISHNA PRIYADARSINI" to "KRISHNA PRIYA PARIDA". Henceforth, She shall be known as "KRISHNA PRIYA PARIDA" for all purposes.
