By virtue of an affidavit sworn before Notary Public Cuttack on Dated 2nd June 2025, I, Mrs. Sumita Jena, Address W/o. Pradip Kumar Jena, At- Balia, P.O.- Taria, P.S- Ranapur, Dist- Nayagarh, Pin- 752026, Odisha, declare that I have changed the name of my Son from "Peasants Jena" to "Prasanta jena". Henceforth, He shall be known as "Prasanta jena" for all purposes.
