By virtue of an affidavit sworn before Executive Magistrate THUAMUL RAMPUR on Dated 17th May 2025, I, Mr. NAGESWAR NAIK, Address AT/POIGUDA, PO/PS - THUAMUL RAMPUR, DIST- KALAHANDI, ODISHA, PIN - 766037, Odisha, declare that I have changed the name of my Son from "DIBYARANJAN NAIK" to "DIBYANSHU NAIK". Henceforth, He shall be known as "DIBYANSHU NAIK" for all purposes.
