By virtue of an affidavit sworn before Executive Magistrate NAYAGARH on Dated 4th June 2025, I, Mr. MANAS CHANDRA BISWAL, Address AT/PO-NOTAR ,PS-ITAMATI ,DIST- NAYAGARH ,PIN-752079, Odisha, declare that I have changed the name of my Daughter from "SONALI BISWAL" to "PRIYADARSINI BISWAL". Henceforth, He shall be known as "PRIYADARSINI BISWAL" for all purposes.
