By virtue of an affidavit sworn before the Executive Magistrate BHUBANESWAR on Dated 13th May 2025, I, Mrs. MENOKA SAMAL, Address AT/PO- Oleichandanapur, PS-Binjharpur, Dist- Jajpur, Pin-755016, Odisha, have changed my name from "MINA SAMAL" to "MENOKA SAMAL". Henceforth, I shall be known as "MENOKA SAMAL" for all purposes.
