SMRUTIRANI PRADHAN, Address TARBOD VILLAGE, PS-KOMNA, PO-TARBOD, DIST-NUAPADA, ODISHA, PIN-766105, Odisha, have changed my name from "SHRIRAMDAS SMRUTI" to "SMRUTIRANI PRADHAN".
