By virtue of an affidavit sworn before Executive Magistrate Erasama on Dated 1st May 2025, I, Mrs. Priyatama Mohanty, Address Village-Gobindapur, Po-Panchapalli, Ps-Erasama, Dist- Jagatsinghpur, Odisha, Pin-754110, Odisha, have changed my Daughter's name from "Soyang Prajna Mohanty" to "Swayanprajna Mohanty". Henceforth, She shall be known as "Swayanprajna Mohanty" for all purposes.
