By virtue of an affidavit sworn before the Executive Magistrate BHUBANESWAR on Dated 1st May 2025, I, Mr. JAGAN MALLICK, Address AT- GAITHIPUR, PO- TERATANGA, PS- BALIKUDA, DIST-JAGATSINGHPUR, BLOCK-BALIKUDA, ODISHA, INDIA, PIN- 754108, Odisha, have changed my name from "JAGANNATH MALLICK" to "JAGAN MALLICK". Henceforth, I shall be known as "JAGAN MALLICK" for all purposes.
