By virtue of an affidavit sworn before Executive Magistrate Tihidi on Dated 28th April 2025, I, Mr. Laxmidhar Barik, Address At-Barsar, Po-Barsar, Ps-Pirahat, Dist-Bhadrak, Odisha, Pin- 756131, Odisha, have changed my Son's name from "Biswajit Barik" to "Subrat Barik". Henceforth, He shall be known as "Subrat Barik" for all purposes.
