By virtue of an affidavit sworn before the Executive Magistrate Bari Jajpur on Dated 31st May 2025, I, Miss. Prangyamita Ojha, Address Village-Bainsiria, Po-Bainsiria, Ps-Bari Ramachandrapur, DistJajpur, Odisha, Pin-755017, Odisha, have changed my name from "Parbati Ojha" to "Prangyamita Ojha". Henceforth, I shall be known as "Prangyamita Ojha" for all purposes.
