By virtue of an affidavit sworn before the Executive Magistrate THUAMUL RAMPUR on Dated 3rd June 2025, I, Mrs. PHULMITA TURUK, Address AT/PO/BIRIKOTE, PS/THUAMUL RAMPUR, DISTRICT - KALAHANDI, PIN - 766037, Odisha, have changed my name from "PHULAMITA NAIK" to "PHULMITA TURUK". Henceforth, I shall be known as "PHULMITA TURUK" for all purposes.
