By virtue of an affidavit sworn before Executive Magistrate Bhubaneswar on Dated 15th February 2025, I, Mr. Sushanta kumar mahanta, Address At- Manoharpur(Bada), Po- Dimirimunda, P.S Sadar, Kendujhar-758014, Odisha, declare that I have changed the name of my Son from "Sushanta Kumar Mahanta" to "S S Mohit Kumar Mahanta". Henceforth, He shall be known as "S S Mohit Kumar Mahanta" for all purposes.
