By virtue of an affidavit sworn before Executive Magistrate Bhubaneswar on Dated 25th September 2025, I, Mrs. Chandana Srimula Venugopal, Address A-18, VIP Colony , Nayapalli , Bhubaneswar, Pin-751012, Odisha, declare that I have changed the name of my Son from "R Nrupen Chakravarthy" to "Nrupen Chakravarthy Ravella". Henceforth, He shall be known as "Nrupen Chakravarthy Ravella" for all purposes.
