By virtue of an affidavit sworn before Executive Magistrate Gop on Dated 28th February 2025, I, Mr. Susant Kumar Dhal, Address Bandha, Arakhapur,Jajpur,754296, Odisha, declare that I have changed the name of my Son's from "Bhagyaraj Dhal" to "Swayam Swastik Dhal". Henceforth, He shall be known as "Swayam Swastik Dhal" for all purposes.
