By virtue of an affidavit sworn before the Executive Magistrate Puri on Dated 8th May 2025, I, Mr. Mukteswara Mallick, Address At-Kapoteswar, Po-Chandanpur, Ps-Chandanpur, Dist- Puri, Odisha, Pin-752012, Odisha, have changed my name from "Sur Mallick" to "Mukteswara Mallick". Henceforth, I shall be known as "Mukteswara Mallick" for all purposes.
