By virtue of an affidavit sworn before Executive Magistrate JAGATSINGHPUR on Dated 16th April 2025, I, Mr. SUSHIL KUMAR RAY, Address AT-KENDAL, PO- MADHUSUDANPUR SASAN, PS/DIST - JAGATSINGHPUR, PIN - 754114,ODISHA, Odisha, declare that I have changed the name of my Daughter from "SWARNIYA RAY" to "SMARANIKA RAY". Henceforth, She shall be known as "SMARANIKA RAY" for all purposes.
