By virtue of an affidavit sworn before Notary Public Bhubaneswar on Dated 17th May 2025, I, Mrs. Sanjukta Biswal, Address W/o Tuna Biswal resident of Village Kamarda, P.O - Kanmarda, P.S - Parjang, Dist Dhenkanal, PIN-759018, Odisha, Odisha, declare that I have changed the name of my Daughter from "Ginjyasha Biswal" to "Jijnasa Biswal". Henceforth, She shall be known as "Jijnasa Biswal" for all purposes.
