By virtue of an affidavit sworn before Executive Magistrate NUAGAON on Dated 6th June 2025, I, Mr. KAMINA NAYAK, Address At-Kosakata,Po-Sampada,Ps-Nuagaon,Dist- Nayagarh,Odisha,Pin-752083, Odisha, declare that I have changed the name of my Daughter from "Saujata nayak" to "Sujata Nayak". Henceforth, She shall be known as "Sujata Nayak" for all purposes.
