By virtue of an affidavit sworn before the Executive Magistrate JAGATSINGHPUR on Dated 23rd May 2025, I, Mr. AYUSH MOHANTY, Address AT-NARADIA(TIKARAPADA), PO-PANCHAPALLI, PS-ERASAMA, DIST-JAGATSINGHPUR, ODISHA, Odisha, have changed my name from "AYUS MOHANTY" to "AYUSH MOHANTY". Henceforth, I shall be known as "AYUSH MOHANTY" for all purposes.
