By virtue of an affidavit sworn before Executive Magistrate THUAMUL RAMPUR on Dated 2nd June 2025, I, Mr. RAI MAJHI, Address AT/BIRMALGUDA, PO/DUMERPADAR, PS/THUAMUL RAMPUR, DISTRICT - KALAHANDI, PIN - 766037, Odisha, declare that I have changed the name of my Son from "SUSANT MAJHI" to "SUSHANTA MAJHI". Henceforth, He shall be known as "SUSHANTA MAJHI" for all purposes.
