By virtue of an affidavit sworn before Executive Magistrate Anandapur on Dated 1st May 2025, I, Mr. Sushanta Kumar Sahoo, Address Village-Mochinda, Po-Salabani, Ps-Anandapur, Dist- Keonjhar, Odisha, Pin-758021, Odisha, declare that I have changed the name of my Daughter from "Shriyasi Sahoo" to "Samaira Sahoo". Henceforth, She shall be known as "Samaira Sahoo" for all purposes.
