By virtue of an affidavit sworn before Executive Magistrate NAYAGARH on Dated 6th June 2025, I, Mr. BRAHMANANDA MOHANTY, Address AT/PO- DIGHIRI, PS- BOLAGARH, DIST- KHORDHA, PIN- 752066, ODISHA, Odisha, declare that I have changed the name of my Daughter from "SWAYAM SHREE PRIYADARSHINI MOHANTY" to "SWOYAMSHREE MOHANTY". Henceforth, She shall be known as "SWOYAMSHREE MOHANTY" for all purposes.
