By virtue of an affidavit sworn before the Executive Magistrate JAGATSINGHPUR on Dated 25th March 2025, I, Mr. DEEPAK DAS, Address AT/PO-TARADAPADA,PS/DIST- JAGATSINGHPUR,ODISHA, Odisha, have changed my name from "JAGANNATH DAS" to "DEEPAK DAS". Henceforth, I shall be known as "DEEPAK DAS" for all purposes.
