By virtue of an affidavit sworn before Executive Magistrate Salipur on Dated 30th May 2025, I, Mr. Chittaranjan Swain, Address At/Po-Malasasan, Ps-Salipur, Dist-Cuttack, Odisha, declare that I have changed the name of my Son from "Swatikas Swain" to "Swastik Swain". Henceforth, He shall be known as "Swastik Swain" for all purposes.
