By virtue of an affidavit sworn before Executive Magistrate Bolagarh on Dated 12th June 2025, I, Mr. Chita Ranjan Pradhan, Address At-Retamaha, Po-Tiangia, Ps-Raikia, Dist-Kandhamal, Odisha, Pin-762100, Odisha, declare that I have changed the name of my Son from "Swayam Prasad Pradhan" to "Chirag Pradhan". Henceforth, He shall be known as "Chirag Pradhan" for all purposes.
