By virtue of an affidavit sworn before the Notary Public Bhubaneswar on Dated 13th November 2025, I, Mrs. Anima Panigrahy, Address Middle Street, Near Mandir, At/ Po - Bipulingi, Chatrapur, Ganjam, Odisha, have changed my name from "Anima Kumari Behera" to "Anima Panigrahy". Henceforth, I shall be known as "Anima Panigrahy" for all purposes.
