By virtue of an affidavit sworn before the Executive Magistrate Bhubaneswar on Dated 8th April 2025, I, Mrs. Sasmita Mansingh, Address At - Maa Mangala Basti , Road No - 8 , Back Side of Siddharth Apartment, Bhoinagar, Bhubaneswar, Dist - Khordha, Odisha, PIN - 751022, Odisha, have changed my name from "Sasmita Baliyarsingh" to "Sasmita Mansingh". Henceforth, I shall be known as "Sasmita Mansingh" for all purposes.
