By virtue of an affidavit sworn before Executive Magistrate Bhubaneswar on Dated 11th December 2025, I, Mr. Sanjay Kumar Baral, Address At- Plot No-701/2001, Bankual,Tankapani Road Back Side of kalpana Apartment, Po-Bankual,Bhubaneswar, dis- khorda, Odisha, declare that I have changed the name of my Daughter from "Sreyanshree Baral" to "Shreyanshi Priyadarshini". Henceforth, She shall be known as "Shreyanshi Priyadarshini" for all purposes.
