By virtue of an affidavit sworn before the Notary Public, Bhubaneswar on the 3rd May 2023, I, Rekha Chandgoita, W/o Nawal Kishore Chandgoita, Resident of Main Road, Titlagarh, Dist. Bolangir, Odisha-767039, have changed my surname from "Rekha Agrawal" to "Rekha Chandgoita". Henceforth, I shall be known as Rekha Chandgoita for all purposes.
