By virtue of an affidavit sworn before the Notary Public, Cuttack on the 11th January 2024, I, Banita Sharma, wife of Ashish Sharma, Permanent Resident of At Laxmi Bazar, Anugul, P.O./P.S. Anugul, Dist. Anugul, Odisha-759122, have changed my surname from "Mukhi" to "Sharma''. Henceforth, I shall be known as Banita Sharma for all purposes.
