By virtue of an affidavit sworn before the Notary Public, Cuttack on the 13th December 2023, I, Ayesha Begum, W/o Imtyaj Khan, permanent resident of Oriyabazar, Myca Lane, P.O. Buxibazar, P.S. Lalbag, Dist. Cuttack, Odisha have changed my name/surname from "Sasmita Barik" to "Ayesha Begum''. Henceforth, I shall be known as Ayesha Begum for all purposes.
