By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar on the 4th January 2023, I, Lopa Mudra Panda, D/o Ashok Kumar Panda, Vill. Mangalpur, P.O./P.S. Mangalpur, Dist. Jajpur, Odisha declare that, I have changed my name from "Sanghamitra Samal" to "Lopa Mudra Panda''. Henceforth, I shall be known as Lopa Mudra Panda for all purposes.
