By virtue of an affidavit sworn before the Notary Public, Sonepur on the 12th January 2024, I, Shibanee Urma, W/o Hinanshu Barik, R/o Vill. Luhurapali, P.O. Rengali, P.S. Dunguripali, Odisha declare that, I have changed my surname from "Shibanee Barik" to "Shibanee Urma''. Henceforth, I shall be known as Shibanee Urma for all purposes.
