By virtue of an affidavit sworn before the Executive Magistrate, Kakatpur on the 6th February 2024, I, Abhay Kumar Bariki, S/o Rama Chandra Bariki, Vill. Upula, P.O. Dhanitri, P.S. Konark-752111, Dist. Puri, Odisha declare that, I have changed my name from "Santosh Kumar Sahoo" to "Abhay Kumar Bariki''. Henceforth, I shall be known as Abhay Kumar Bariki for all purposes.
