By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar on the 6th February 2024, I, Parbati Samal, W/o Madhab Bhoi, Vill. Pandasahi, P.O./P.S. Sasan, P.S. Balianta, Dist. Khurda, Odisha declare that, I have changed my name from "Gayatri Bhoi" to "Parbati Samal''.
Henceforth, I shall be known as Parbati Samal for all purposes.
