By virtue of an affidavit sworn before the Notary Public, Bhubaneswar on the 9th February 2024, I, Dileep Kumar Sahoo staying at Plot No. 347, Near Bishnupriya Apartment, Jaydev Vihar, P.S. Jaydev Vihar, Bhubaneswar-751013, Dist. Khurda, have changed my name from "Gudia Dileep Kumar" to "Dileep Kumar Sahoo''. Henceforth, I shall be known as Dileep Kumar Sahoo for all purposes.
