By virtue of an affidavit sworn before the Notary Public, Bhubaneswar on the 10th February 2024, I, Perpety Banamali Reddy, Vill. Bellaguntha, Reddy Street, Bhanjanagar, Dist. Ganjam, Odisha declare that, I have changed my son's name from "P. Siddharth" to "Perpety Siddharth Reddy''.
Henceforth, my son shall be known as Perpety Siddharth Reddy for all purposes.
