No. B/177, Sector-7, P.O. Rourkela-03, P. S. Sector-7, Dist. Sundargarh, Odisha declared that, I have changed my surname from "Patra'' to "Mohanty''. Henceforth, I shall be known as Puspanjali Mohanty for all purposes.
