By virtue of an affidavit No. 69AA 280235, dated the 2nd February 2024 before the Notary Public, Bhubaneswar, I, Anchal Ashmita, D/o Phakir Mohan Sahu, W/o Tanmoy Dash, At Flat No. 1002, Tower-3, Vipul Gardens, Shankarpur, Ghatikia, P.S. Chandaka, Bhubaneswar-751003, have changed my name from Anchal Ashmita to Anchal Ashmita Dash. Henceforth, I shall be known as Anchal Ashmita Dash for all future purposes.
